About The Position
All Saints Subacute & Transitional Care is seeking a Full-Time Licensed Vocational Nurse (LVN) to join their clinical team. This role involves working with a diverse patient population in a specialized subacute and transitional care setting. The position offers a stable, high-energy environment with a focus on clinical excellence and patient outcomes.
Requirements
- Must possess a current, valid California Licensed Vocational Nurse (LVN) license.
- Current CPR/BLS Certification.
- Strong clinical judgment.
- Excellent communication skills.
- A "team-first" attitude.
Nice To Haves
- Previous experience in subacute, skilled nursing, or transitional care is preferred.
- PointClickCare experience is highly valued.
Responsibilities
- Provide high-quality nursing care, including medication administration, wound treatments, and resident assessments.
- Closely monitor resident vitals and conditions, promptly reporting any changes to the RN or physician.
- Collaborate with the in-house therapy team (PT/OT/SLP) and physicians to ensure comprehensive recovery plans.
- Maintain precise, timely, and compliant electronic medical records (PointClickCare experience is highly valued).
- Provide direction and mentorship to CNAs to ensure resident ADLs are met with the highest standards of care.
- Act as a compassionate resource for residents and their families during their transitional care journey.
