About The Position

At Victorian Post Acute, we are committed to delivering exceptional skilled nursing and rehabilitation services in a compassionate and resident-centered environment. We are seeking a dedicated Licensed Vocational Nurse (LVN/LPN) to join our clinical team on the full-time AM shift (7:00 AM - 3:30 PM).

Requirements

Current, active, and unrestricted California LVN/LPN license.
Graduate of an accredited LVN/LPN nursing program.
Strong clinical, communication, and leadership skills with a commitment to resident care excellence.

Nice To Haves

Long-term care, skilled nursing, or post-acute experience preferred.

Responsibilities

Provide direct nursing care and medication administration in accordance with physician orders and resident care plans.
Supervise and support CNAs to ensure quality resident care and compliance with regulatory standards.
Monitor resident conditions, document clinical updates, and communicate changes to physicians and families as needed.
Assist with admissions, discharges, treatments, assessments, and interdisciplinary care coordination.
Direct daily nursing activities and ensure adherence to facility policies and procedures.
Maintain accurate resident records, treatment documentation, and physician orders.
Complete incident reports and participate in quality assurance initiatives.
Collaborate with nursing leadership to ensure smooth shift operations and resident-centered care.
Follow all federal, state, and facility regulations, including HIPAA and infection control standards.
Ensure proper medication management, narcotic accountability, and treatment administration.
Promote a safe, clean, and sanitary environment for residents and staff.
Participate in facility surveys, safety drills, and required in-service training programs.
