About The Position
Tri-FLEXSI Home Health Care, Inc. is seeking a compassionate and skilled Licensed Vocational Nurse (LVN) to join our team in Missouri City, Texas. The LVN will work full time and provide high-quality, compassionate care to our patients in their own homes. This individual contributor role will report directly to the Director of Nursing and will work closely with a team of healthcare professionals to provide comprehensive home health services. The ideal candidate will have a strong background in healthcare and a passion for delivering exceptional patient care.
Requirements
- Current and valid Licensed Vocational Nurse (LVN) license in the state of Texas
- Minimum of 1 year of experience in healthcare, preferably in a home health setting
- CPR certification
- Excellent clinical skills and knowledge of nursing practices and procedures
- Empathetic, compassionate, and patient-centered approach to care
- Ability to work independently and as a part of a team
- Strong communication and interpersonal skills
- Proficiency in electronic health records (EHR) systems
- Must have a reliable mode of transportation and a valid driver's license
Responsibilities
- Provide compassionate and skilled nursing care to patients in their homes, following the established care plan
- Administer medications, dress wounds, and assist with other medical treatments as directed by the physician
- Monitor and record patient’s vital signs, progress, and any changes in condition
- Collaborate with the physician and other members of the healthcare team to develop and update patient care plans
- Educate patients and their families on medication management, disease prevention, and self-care techniques
- Maintain accurate and complete patient records in accordance with state and federal regulations
- Communicate effectively with patients, their families, and healthcare team members
- Adhere to all company policies, procedures, and protocols
