About The Position

Frontpoint Health is a fast-growing home health agency committed to delivering personalized, high-quality care. We're currently seeking experienced and compassionate Licensed Vocational Nurses (LVN) to join our team and help patients recover safely and comfortably in their homes. We are seeking a skilled and compassionate LVN to join our team. The LVN will be responsible for providing nursing care to patients in their homes, under the direction of a registered nurse or physician. The ideal candidate will have experience in home health care, excellent communication skills, and a passion for helping others.

Requirements

• Current LVN license in the state of Texas
• Minimum of one year of experience in home health care
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a team
• Strong critical thinking and problem-solving skills
• Valid driver's license and reliable transportation
• Ability to pass a background check and drug screening
• Wound care 2-3 x week

Responsibilities

• Assess patients' health status and develop care plans
• Administer medications and treatments as prescribed by a physician
• Monitor patients' vital signs and report any changes to the RN or physician
• Provide wound care and other nursing interventions as needed
• Document patient care and progress in electronic medical records
• Communicate with patients, families, and other healthcare providers as needed
• Participate in interdisciplinary team meetings and case conferences
• Adhere to all state and federal regulations and agency policies and procedures
