About The Position

Under limited supervision, the Licensed Vocational Nurse (LVN) will perform triage of client needs per protocol as needed; prepare patient assist medications for dispensing to patients; process new chronic pain clients per protocol; participate in referral processes including tracking; assist the medical team with client care in the clinic as needed; assist the RN with dispensary services as needed; assist upper management with quality assurance; and contribute to planning and implementing strategic goals. The role involves performing face-to-face or telephone triage, consulting with RN or Clinician for advice or instructions, providing patient education, assisting in the orientation of new staff, following up on referrals, and autoclaving instruments. Case management is also a key responsibility. The LVN must adhere to policies and procedures, participate in team systems, perform duties ethically, maintain client confidentiality, demonstrate the ability to re-prioritize duties, use time efficiently, be responsible for co-worker and client satisfaction, display flexibility, maintain a neat, clean, and safe work environment, and perform job duties in a quality manner.

Requirements

  • Current licensure in California as LVN.
  • Knowledge of standard nursing practices and processes of RN’s, LVN’s, MA’s and other nursing personnel.
  • Current CPR certification.
  • Blood Withdrawal Certification.
  • Current driver's license and insurance.
  • Intermediate level language skills.
  • Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.
  • Ability to write routine reports and correspondence.
  • Ability to speak effectively before individuals, groups or employees of organization.
  • Basic math skills.
  • Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.
  • Ability to compute rate, ratio, and percentages.
  • Very high level reasoning skills.
  • Ability to define problems, collect data, establish facts, and draw valid conclusions.
  • Ability to interpret an extensive variety of technical information and deal with several abstract and concrete variables.
  • Basic working knowledge of Windows operating systems, Microsoft Office Suite - Word, Excel, Outlook, and Internet browsers.
  • Working experience with medical management software such as Health Pro and Medi-Tracks.
  • Good working knowledge of standard nursing equipment.
  • Ability to work independently and with others.

Responsibilities

  • Perform triage of client needs per protocol as needed.

  • Document receipt of medication in patient chart after checking for correct dosage and directions.
  • Label each medication appropriately and put in dispensary for patient pick-up; notify patient.
  • Verify new pain patient according to pain management protocol.
  • Verify patient’s charts to assure continuous flow of care.
  • Verify patient medication utilizing the 5 rights of medication.
  • Perform face to face or telephone triage, consult with RN or Clinician for advice or instructions to be given.
  • Provide patient education.
  • Assist in orientation of new staff.
  • Follow up of referrals.
  • Autoclave instruments.
  • Perform Case Management.
