Licensed Vocational Nurse
About The Position
Merakey is seeking a Licensed Vocational Nurse / Licensed Practical Nurse to join their team in Escondido, CA. This is a 2nd shift position, Sunday-Thursday, 3pm-11pm. The LVN will provide direct nursing care under the supervision of a Registered Nurse, ensuring the implementation of physician orders, coordination of medical appointments, and continuity of care for individuals with complex medical needs. This role operates in a structured, team-based environment where nurses are supported and empowered to deliver high-quality care. The position is responsible for the overall medical needs of assigned individuals in their Intellectual Developmental Disability residential programs.
Requirements
- LVN/LPN Active CA license in good standing
- Valid driver's license.
- Strong organizational and documentation skills
- Ability to work collaboratively in a team environment
Nice To Haves
- 1 year experience in long-term care, developmental services, or skilled nursing preferred
Responsibilities
- Implement program policy and procedure according to program regulations and guidelines.
- Provides overall medical support to individuals who receive services to include scheduling medical appointments and follow-up as needed.
- Works closely with Program Director and Administrator to ensure health and safety of individuals receiving services.
- Provides on-site medical support to individuals.
- Represent Merakey at all external meetings, conferences, etc. as assigned.
- Be knowledgeable and adhere to program regulations, assisting in preparation for annual licensure as applicable.
- Maintain frequent contact with family members, as deemed appropriate.
- Maintain frequent contact with external stakeholders as needed.
- May perform other related duties when assigned or required.
