About The Position

Merakey is seeking a PRN Licensed Vocational Nurse (LVN) to join their team in Yuba City, CA. This role involves coordinating services and care in a community-based program, performing nursing duties including assessment, treatment, and resolution of health issues under the supervision of an RN. Responsibilities include managing medical needs of assigned individuals, providing insulin and following diabetic orders, chart and MAR reviews, medication reorders, participation in community outings and appointments, collaboration with families and teams, supporting care plans, providing Type 1 diabetes management, and scheduling medical appointments.

Requirements

• Active and unrestricted CA LVN license.
• Valid driver's license.
• Strong communication and teamwork skills.
• Ability to work independently and collaboratively.

Nice To Haves

• Behavioral health or residential experience.
• Type 1 diabetes management experience.
• Insulin administration.
• Familiarity with Dexcom continuous glucose monitoring system.
• Developmental disability services.

Responsibilities

• Providing insulin and following diabetic orders.
• Chart and MAR reviews.
• Medication reorders and coordination with pharmacies/providers.
• Participation in community outings and appointments.
• Collaboration with families, conservators, and IDT team members.
• Support implementation of behavioral and medical care plans.
• Provide Type 1 diabetes management
