About The Position

Merakey is seeking a Licensed Vocational Nurse (LVN) to join their team in Yuba City, CA. This role involves coordinating services and care in a community-based program, working under the supervision of an RN. The LVN will be responsible for the overall medical needs of assigned individuals, performing nursing duties such as assessment, treatment, and follow-through for all health issues. Specific duties include providing insulin and following diabetic orders, chart and MAR reviews, medication reorders and coordination, participation in community outings and appointments, collaboration with families and teams, supporting care plans, providing Type 1 diabetes management, and scheduling medical appointments.

Requirements

• Active and unrestricted CA LVN license.
• Valid driver's license.
• Strong communication and teamwork skills.
• Ability to work independently and collaboratively.

Nice To Haves

• Behavioral health or residential experience.
• Type 1 diabetes management experience.
• Insulin administration.
• Familiarity with Dexcom continuous glucose monitoring system.
• Developmental disability services.

Responsibilities

• Providing insulin and following diabetic orders.
