We are seeking a dedicated Licensed Vocational Nurse (LVN) to support our residential crisis programs. Requirements

  • Current California Licensed Vocational Nurse (LVN) license
  • Ability to work effectively within scope of practice and under RN supervision
  • Strong clinical documentation skills
  • Valid driver's license and reliable transportation (if required for appointments)

Responsibilities

  • Implement nursing interventions as outlined in physician and RN-directed care plans.
  • Monitor client health status and document observations, medication responses, behavioral changes, and health-related events.
  • Measure and document weekly vital signs and weights for assigned clients.
  • Identify and immediately report significant changes in client condition to the supervising RN.
  • Administer medications according to physician orders and current Medication Administration Records (MARs).
  • Complete medication documentation accurately and timely.
  • Conduct medication audits as assigned.
  • Reconcile medications following physician appointments or prescription changes.
  • Notify the RN immediately regarding any medication changes, discrepancies, or errors.
  • Schedule and coordinate medical, psychiatric, and specialty appointments.
  • Accompany clients to appointments as needed.
  • Ensure all required documentation and forms are completed.
  • Submit appointment summaries and provider recommendations to the RN within 24 hours.
  • Maintain accurate client health records.
  • Submit updated MARs monthly and whenever changes occur.
  • Forward provider correspondence, treatment notes, and recommendations to the RN.
  • Ensure all clinical documentation is completed on the day services are provided.
  • Deliver RN-developed training programs to residential staff.
  • Support Direct Support Professionals (DSPs) with health-related tasks.
  • Escalate clinical concerns beyond LVN scope to the RN.
  • Collect training attendance documentation and submit to nursing leadership.
  • Respond appropriately to emergencies, including activating emergency medical services when necessary.
  • Notify leadership and supervising RN promptly.
  • Complete required documentation following incidents.
  • Assist with hygiene-related support as assigned.
  • Assist with household tasks and residential operations as assigned.
  • Perform other duties necessary to support quality client care and program operations.
  • The LVN must contact the RN the same day for: Any medication change, Medication errors or MAR discrepancies, Emergency room visits, hospitalizations, or 911 activations, New physician or psychiatric orders, Changes in client condition, symptoms, or behavior, Significant medical or psychiatric appointment findings, Provider communications containing clinical information, Any situation exceeding LVN scope of practice or requiring clinical guidance.
