About The Position

Concord Post Acute is seeking a compassionate, dependable, and dedicated Licensed Vocational Nurse (LVN) to provide high-quality, resident-centered care in a supportive environment. This is a part-time position paying $38.00 per hour. The shift is to be determined.

Requirements

• Hold a current and active California LVN license.
• Have knowledge of nursing practices, medication administration, and infection prevention.
• Demonstrate strong communication, organizational, and clinical skills.
• Be able to work effectively as part of a multidisciplinary team.
• Bring a compassionate, professional, and resident-focused approach to care.

Nice To Haves

• Previous skilled nursing or post-acute care experience.

Responsibilities

• Provide nursing care to residents in accordance with physician orders and established care plans.
• Administer medications and treatments as prescribed.
• Monitor residents’ health status and promptly report changes to appropriate healthcare professionals.
• Accurately document nursing assessments, treatments, medications, and resident care.
• Help coordinate resident care and communicate with physicians, families, and interdisciplinary team members.
• Follow infection control procedures, safety standards, and facility policies.
• Support resident dignity, comfort, and overall well-being.
• Assist with admissions, transfers, and discharges as needed.
