Requirements

• Licensed Vocational Nurse (LVN) license
• PRN Home Health experience

Responsibilities

• Ability to administer skilled nursing care for clients of all ages in their place of residence under the direction of a Registered Nurse.
• Coordinate care with the interdisciplinary team, patient/family and referring agency.
• Participates in planning and implementing care in partnership with the patient, representative (if any), caregiver(s), and the RN.
• Provides services that are ordered by the physician as indicated in the plan of care.
• Provides patient and caregiver education.
