About The Position

The Licensed Vocational Nurse (LVN) - Pediatrics - Evenings/Weekends is responsible for performing a variety of duties in collaboration with clinical and administrative staff to deliver optimal patient care services within the scope of the LVN's professional expertise. This role ensures patients have a positive healthcare experience, with their care managed and coordinated effectively, and that patients participate in decision-making. As a member of the clinical team, this position requires full support of the Patient-Centered Medical Home (PCMH) principles, policies, and procedures. The LVN will assist with patient preparation, consider the age of the patient during assessments, and may occasionally assist with charge duties or provide guidance to clinical staff. The LVN will also be expected to work at other clinic sites as needed in the absence or need of a provider.

Requirements

  • High School or GED.
  • Must be a licensed vocational nurse in the State of Texas.
  • Must be computer literate.
  • Must be current with BLS/CPR.
  • Bilingual (English/Spanish) a must.
  • Ability to complete basic/intermediate math computations and medical math conversions.
  • Ability to examine documents for accuracy and completeness.
  • Ability to maintain an effective and professional working relationship with the public and co-workers.
  • Ability to maintain client, office confidentiality and abides by confidentiality expectations and HIPAA guidelines.
  • Ability to prepare records in accordance with detailed instructions.
  • Ability to read, write and speak in a clear, accurate and professional manner; includes active listening skills and understanding medical terminology.
  • Ability to recognize, evaluate, solve problems, correct errors, and report to supervisors of issues.
  • Ability to resolve patient service issues and exercise independent judgment.
  • Ability to work in a fast-paced environment.
  • Able to bend, stretch, stoop, stand, sit, and lift 20 lbs.
  • Able to insert urine catheters into a specific patient population.
  • Ability to communicate concerns to appropriate personnel using tact and diplomacy.
  • Does not abuse sick leave or other periods allowed away from the job.
  • Ability to interpret and translate for the provider when necessary.

Responsibilities

  • Actively and accurately record patient information, including medication updates and summaries from the patient pre-visit summary, in the medical record.
  • Participate in multidisciplinary team huddles to coordinate care, scheduling, and follow-up with providers and front desk clerks.
  • Review opportunities for same-day access and alert other disciplines for patients with and without special needs.
  • Perform and monitor staff administering medication and vaccines, and monitor patients for drug reactions.
  • Attend and complete required in-service trainings, such as HEALH EDU, Su Clinica trainings, and other web-based courses and educational programs.
  • Ensure personal safety and the safety of others.
  • Submit required documentation in a timely manner, including credentialing requirements, license renewals, and certifications.
  • Attend work on a regular and predictable schedule and perform other duties as assigned.
  • Prepare, instruct patients, and collect and process specimens for laboratory testing, alerting the provider of any patient concerns or abnormal results.
  • Complete required electronic documents to coordinate refills and order diagnostic studies/exams through internal or external referral processes.
  • Conduct appropriate screening tests (e.g., glucometers, spirometer) and follow up with the provider on test results.
  • Engage patients and their caregivers in understanding their care in a culturally and linguistically appropriate manner.
  • Observe patients for changes in condition and promptly inform providers of any urgent situations.
  • Participate in quality improvement projects aimed at improving patient care outcomes.
  • Provide training/education to Su Clinica employees or students when requested.
  • Perform patient call backs in a timely manner and document.
  • Perform duties within the scope of practice as delegated by the provider through approved standing orders and/or protocols.
  • Perform nurse coordinator duties by assessing walk-in patients and checking provider schedule availability.
  • Prepare exam rooms, prepare patients for examination, treatment, and procedures, and assist the provider.
  • Take vital signs, medical history, and accurately enter information into the electronic medical record.
  • Teach methods of health promotion and disease prevention in accordance with individualized needs.
  • Refer patients to appropriate resources, such as social services, pharmacy, and laboratory.
  • Verify that patients and family members' discharge and follow-up questions have been addressed and appropriate educational materials provided and documented.
  • Work jointly with providers to maximize disease prevention and reinforce patient follow-through with provider orders and instructions.
  • Perform patient testing and reporting as assigned and perform laboratory tests as described in the Laboratory Manual.
  • Perform routine waived tests with accuracy and precision under minimal/no supervision.
  • Maintain adequate stock levels.
  • Run controls, monitor QC data, and perform/document proficiency tests.
  • Keep all laboratory records on file and assist in laboratory inspection preparedness.
  • Maintain instruments following manufacturer recommendations.
  • Document accurately, maintaining action logs.
  • Supervise new staff in the performance of procedures.
  • Assist with the control of the department budget.
  • Communicate concerns to appropriate personnel using tact and diplomacy.
  • Demonstrate role in safety rules and emergency preparedness.
  • Initiate prompt and accurate treatment, diagnostic screening, and medications as ordered.
