They are a leading short-term rehab and skilled nursing facility committed to helping residents regain their independence through elite, personalized care. The LVN will drive the recovery process through high-level nursing oversight and compassionate care, delivering tailored nursing services, exercising advanced critical thinking, managing precise clinical documentation, and executing physician orders with accuracy.

Requirements

• Must possess an active, valid California LVN license.
• Proven experience in geriatric, post-acute, or skilled nursing rehabilitative care is required.
• Minimum of 2 years of Skilled Nursing Facility (SNF) experience.

Responsibilities

• Deliver tailored nursing services designed to meet each resident's unique recovery milestones.
• Exercise advanced critical thinking to make independent decisions that directly improve patient outcomes.
• Manage precise clinical documentation and serve as a primary liaison between families and the interdisciplinary medical team.
• Execute physician orders and complex clinical instructions with total accuracy and attention to detail.
• Act as a reliable, experienced anchor for our residents and a resource for the nursing team during the AM shift.
