In this role, you will provide high‑quality direct patient care under the supervision of a Registered Nurse (RN). You will follow individualized care plans, administer treatments, document clinical information, and collaborate with the interdisciplinary care team to support positive patient outcomes .
Requirements
- Current LVN license in California.
- Minimum of 1 years nursing experience preferred (home health or medical/surgical experience strongly valued).
- Knowledge of professional nursing standards and clinical best practices.
- Strong verbal, written, and organizational communication skills.
- Reliable transportation for home visits; valid driver’s license and current auto insurance required.
- Current CPR certification .
- Ability to lift, position, and transfer patients; requires frequent standing, walking, bending, and reaching.
Responsibilities
- Provide direct patient care in accordance with the State Nurse Practice Act.
- Carry out the plan of care developed by the Registered Nurse.
- Complete accurate, timely, and compliant clinical documentation .
- Perform patient observations and collect assessment data to report to the RN or physician.
- Communicate the need for additional home health services (HHA, OT, PT, MSW, ST, Dietician) to the clinical team.
- Utilize medical equipment and supplies safely, efficiently, and according to agency guidelines.
- Participate in ongoing professional development and skills training.
