About The Position

Avalon View Health and Wellness is seeking compassionate and dependable Licensed Practical Nurses to join our nursing team in Liberty, Missouri. As an LPN at Avalon View, you will provide direct nursing care, monitor resident conditions, administer medications and treatments, and help coordinate care throughout your assigned shift. You will work closely with residents, families, nursing assistants, physicians, therapy professionals, and other members of the interdisciplinary care team. The Licensed Practical Nurse is responsible for providing safe, efficient, and compassionate nursing care while supporting positive clinical outcomes and resident and family satisfaction. The LPN practices within the authorized scope of the Missouri Nurse Practice Act and follows accepted professional standards, physician orders, facility policies, and applicable state and federal regulations. The LPN also assists with directing nursing care and supporting nursing personnel during the assigned shift.

Requirements

  • Graduate of an approved practical nursing program
  • Current, active Licensed Practical Nurse license in the State of Missouri
  • Current BLS/CPR certification
  • Strong clinical judgment, communication, leadership, and documentation skills
  • Dependable attendance and a strong work ethic
  • Ability to adapt when resident needs or priorities change
  • Ability to work collaboratively with an interdisciplinary care team
  • Professional and compassionate demeanor
  • Commitment to treating every resident with compassion, dignity, and respect

Nice To Haves

  • At least one year of skilled nursing or long-term care experience preferred

Responsibilities

  • Provide nursing care according to individualized care plans, physician orders, professional standards, and applicable regulations
  • Administer medications and treatments safely and accurately
  • Complete medication administration, treatment, and clinical documentation within required timeframes
  • Assess and monitor residents within the LPN’s authorized scope of practice
  • Identify and promptly report changes in resident condition to the appropriate supervisor, physician, and responsible party
  • Assist with resident admissions, transfers, and discharges, including required documentation
  • Participate in welcoming and orienting new residents and their families to the community
  • Coordinate care with nursing assistants, physicians, therapy professionals, pharmacy personnel, and other members of the interdisciplinary team
  • Provide direction, education, and support to nursing personnel during the assigned shift
  • Serve as a clinical resource for nursing staff within the authorized scope of practice
  • Accurately identify and report skin changes and follow established skin-management protocols
  • Obtain and report diagnostic test results in a timely manner
  • Count controlled medications at the beginning and end of each shift according to facility procedures
  • Follow pharmacy procedures for ordering, receiving, administering, storing, and documenting medications
  • Document incidents, accidents, adverse events, grievances, and concerns according to facility policy
  • Participate in facility surveys and inspections as assigned
  • Follow infection prevention, safety, quality-of-care, and restraint-reduction procedures
  • Protect resident rights, privacy, dignity, and confidentiality
  • Respond calmly and appropriately to changes in resident condition and other clinical situations
