We are a leading short-term rehab and skilled nursing facility committed to helping residents regain their independence through elite, personalized care.
Requirements
- Must possess an active, valid California LVN license.
- Proven experience in geriatric, post-acute, or skilled nursing rehabilitative care is required.
Responsibilities
- Deliver tailored nursing services designed to meet each resident's unique recovery milestones.
- Exercise advanced critical thinking to make independent decisions that directly improve patient outcomes.
- Manage precise clinical documentation and serve as a primary liaison between families and the interdisciplinary medical team.
- Execute physician orders and complex clinical instructions with total accuracy and attention to detail.
- Act as a reliable, experienced anchor for our residents and a resource for the nursing team during the AM shift.
