About The Position

Soleo Health is seeking a Per Diem Licensed Vocational Nurse (LVN) to support our Specialty Infusion patients in Webster, TX. The Licensed Vocational Nurse (LVN) provides primary patient care for specifically assigned patients to include assessments, care planning, evaluation and education of patients receiving infusion therapy and/or other nursing services in accordance with the care matrices and best practices in the home or alternate site setting. This role involves providing direct care, coordinating with various care team members, evaluating and educating patients on home infusion therapy, assessing the home environment for safety, developing Care Maps, managing caseload scheduling, documenting communications, and assisting with vital sign monitoring, PICC dressing changes, PIV insertions, and administrative needs as permitted by state nursing laws.

Requirements

  • Graduate of an accredited LPN or LVN nursing program
  • Current LVN license for applicable state in good standing
  • IV certification required
  • Must hold current IV certification in accordance with state and organizational standards.
  • Minimum 3 years nursing experience with background in infusion therapy
  • Must have knowledge and have used IV access devices to include: IV lines, accessing ports, and administration of infusion medications
  • Must be licensed as an LVN in the state of Texas
  • Excellent customer service skills
  • Proficient with Microsoft Excel, Word and Outlook

Nice To Haves

  • IVIG Infusion experience preferred
  • Experience in ICU and/or Cardiac Care preferred

Responsibilities

  • Provide the majority, if not all the direct care for all specifically assigned patients including initiation and maintenance of all communications with appropriate care team members including pharmacists, physicians, dieticians, home patient representatives and any appropriate community resource staff members.
  • Evaluate and education patient/care partner to safely administer the home infusion therapy and provide additional instruction as indicated.
  • Evaluate the home environment where the care is to be provided and identify interventions that may be required to render the home care environment safe and appropriate for the prescribed care.
  • Develop, in conjunction with other clinical team members, each patient’s Care Map and evaluate patient’s response to therapy during each home visit or encounter.
  • Appropriately discharge primary patients and complete discharge summary within 7 days of discharge.
  • Develop and maintain caseload scheduling and communications on a weekly and monthly basis in updating schedule with changes.
  • Schedule home visits with patient/care partner and document on patient visit schedule calendar, which is posted in patient’s home.
  • Document patients visit to physician or lab drawing station on calendar.
  • Coordinate care via live telephone conversations and/or voicemail with the team as determined by the urgency of information to be relayed.
  • Document all communications at the time of service.
  • Assist with vital sign monitoring, PICC dressing changes, PIV insertions, and with administrative needs depending on site of care and state nursing laws.
