About The Position

Assisted is experiencing tremendous growth and is looking for a LVN for their Baldwin Park patients. Voted #1 Home Health & Hospice Agency by Ventura County Star! This role involves working one-on-one with an assigned patient, ensuring quality and safe delivery of home health nursing services. You will also communicate with the RN Case Manager and physician regarding patient condition and needs, and assist the RN in carrying out the plan of care. Full-time 40-hour day or night shifts are available.

Requirements

• Graduate of accredited practical (vocational) nursing program
• Licensed as an LVN by the California Board of Vocational Nurse and Psychiatric Technician Examiners

Responsibilities

• Work one-on-one with an assigned patient
• Ensure quality and safe delivery of home health nursing services
• Communicate with RN Case Manager and physician, as appropriate, on a regular basis regarding changes in the patient's condition and/or needs
• Assist the RN in carrying out the plan of care
