About The Position

Concho Hearts Hospice is seeking a compassionate Licensed Vocational Nurse (LVN) to join their team serving the Washington and surrounding area. This role focuses on providing compassionate nursing care to hospice patients in their homes or care facilities, building relationships, and making a difference during meaningful moments. The position offers flexibility with options for full-time or PRN schedules.

Requirements

• Active Texas LVN license.
• Strong communication and organizational skills.
• Compassionate, dependable, and patient-focused.
• Reliable transportation and willingness to travel locally.
• Current CPR certification.

Nice To Haves

• Hospice, home health, long-term care, or clinical nursing experience preferred.

Responsibilities

• Provide compassionate nursing care to hospice patients in their homes or care facilities.
• Complete routine patient visits and assist with admissions as assigned.
• Monitor patient conditions and communicate changes to the RN Case Manager and interdisciplinary team.
• Administer medications and treatments as ordered.
• Educate patients and families on disease progression, comfort measures, and medications.
• Maintain accurate, timely documentation using a company-provided tablet.
• Work closely with physicians, RNs, CNAs, Social Workers, Chaplains, and other team members to provide exceptional patient care.
