About The Position

Requirements

  • Current Licensed Vocational Nurse (LVN) license in the State of California required.
  • Valid California Driver License, clean driving record, and insurability under Alcott’s insurance policy required.
  • Live Scan fingerprint clearance required.

Nice To Haves

  • One to three (1–3) years of clinical experience working with individuals with serious mental illness (SMI), homelessness, and/or complex populations preferred.
  • Experience in behavioral health, social services, supportive housing, or community-based care settings strongly preferred.
  • Current CPR Certification preferred.
  • Strong understanding of confidentiality standards and HIPAA requirements.
  • Ability to perform effectively in high-stress situations and work with clients experiencing behavioral health crises or emotional distress.
  • Strong organizational, communication, and care coordination skills.
  • Familiarity with Electronic Health Records (EHR) and related healthcare technology systems.
  • Ability to work collaboratively within a multidisciplinary team environment.

Responsibilities

  • Provide basic nursing services, including injections, medication administration, and monitoring.
  • Coordinate and schedule primary care and specialty medical appointments for clients.
  • Maintain medication logs in collaboration with the Program Manager for clients receiving psychiatry services through community partners.
  • Coordinate with pharmacies to ensure timely medication delivery and continuity of care.
  • Provide patient education and supportive services to promote medication compliance and wellness.
  • Work collaboratively with an interdisciplinary team to support clients with complex medical, mental health, and substance use needs.
  • Communicate effectively with internal staff, external providers, pharmacies, and community partners to ensure continuity of care.
  • Provide crisis intervention support and coordinate emergency response services when necessary.
  • Participate in weekly staff meetings, case conferences, and supervision sessions.
  • Maintain accurate and timely documentation within the designated electronic health record or charting system.
  • Support clients in navigating Medi-Cal benefits and healthcare systems.
  • Maintain compliance with HIPAA, confidentiality standards, and agency policies and procedures.
  • Assist with day-to-day program operations and additional duties assigned by leadership to support quality client care and program effectiveness.
