Requirements
- Current Licensed Vocational Nurse (LVN) license in the State of California required.
- Valid California Driver License, clean driving record, and insurability under Alcott’s insurance policy required.
- Live Scan fingerprint clearance required.
Nice To Haves
- One to three (1–3) years of clinical experience working with individuals with serious mental illness (SMI), homelessness, and/or complex populations preferred.
- Experience in behavioral health, social services, supportive housing, or community-based care settings strongly preferred.
- Current CPR Certification preferred.
- Strong understanding of confidentiality standards and HIPAA requirements.
- Ability to perform effectively in high-stress situations and work with clients experiencing behavioral health crises or emotional distress.
- Strong organizational, communication, and care coordination skills.
- Familiarity with Electronic Health Records (EHR) and related healthcare technology systems.
- Ability to work collaboratively within a multidisciplinary team environment.
Responsibilities
- Provide basic nursing services, including injections, medication administration, and monitoring.
- Coordinate and schedule primary care and specialty medical appointments for clients.
- Maintain medication logs in collaboration with the Program Manager for clients receiving psychiatry services through community partners.
- Coordinate with pharmacies to ensure timely medication delivery and continuity of care.
- Provide patient education and supportive services to promote medication compliance and wellness.
- Work collaboratively with an interdisciplinary team to support clients with complex medical, mental health, and substance use needs.
- Communicate effectively with internal staff, external providers, pharmacies, and community partners to ensure continuity of care.
- Provide crisis intervention support and coordinate emergency response services when necessary.
- Participate in weekly staff meetings, case conferences, and supervision sessions.
- Maintain accurate and timely documentation within the designated electronic health record or charting system.
- Support clients in navigating Medi-Cal benefits and healthcare systems.
- Maintain compliance with HIPAA, confidentiality standards, and agency policies and procedures.
- Assist with day-to-day program operations and additional duties assigned by leadership to support quality client care and program effectiveness.
