Licensed Vocational Nurse (LVN) – Private Duty Nursing
About The Position
Guardian Angel Home Care is seeking a compassionate, dependable, and experienced Licensed Vocational Nurse to provide private duty nursing care to a patient in the San Jose, California area. The role provides dedicated, one-on-one skilled nursing care in the patient's home and involves working independently while building a positive relationship with the patient and family. The schedule is Monday through Friday, 11:00 AM to 5:00 PM, for 30 hours per week. The stated start date is October 19, 2026. Pay is $45–$50 per hour, depending on experience. Clinical leadership and administrative support are available, and clinical documentation is completed through Homecare Homebase (HCHB).
Requirements
- Active California Licensed Vocational Nurse (LVN) license in good standing.
- Graduate of an accredited vocational nursing program.
- Current BLS/CPR certification.
- Previous nursing experience.
- Strong communication and patient care skills.
- Ability to work independently in a private home setting.
- Dependable and available for the assigned weekday schedule.
Nice To Haves
- Previous private duty nursing or home health experience.
- Experience providing one-on-one patient care.
- Familiarity with electronic medical documentation systems.
- Strong medication administration and patient assessment skills.
Responsibilities
- Provide dedicated one-on-one skilled nursing care to a patient in their home.
- Monitor the patient's condition, vital signs, and overall health status.
- Administer medications and treatments as prescribed and within the LVN scope of practice.
- Assist with activities of daily living and patient care needs as appropriate.
- Observe and promptly report changes in the patient's condition to the supervising RN and physician.
- Maintain accurate and timely clinical documentation.
- Communicate professionally with the patient, family, and interdisciplinary care team.
- Follow the established plan of care and all applicable agency and state regulations.
- Maintain a safe, comfortable, and supportive environment for the patient.
