About The Position
Sunnyvale Post Acute is seeking a dedicated Licensed Vocational Nurse (LVN) to join our clinical team in Sunnyvale, CA. We pride ourselves on providing high-quality, compassionate care to our residents in a supportive and collaborative environment. This role involves providing direct nursing care, administering medications, monitoring residents, documenting care, supervising CNAs, and assisting with resident transitions. The ideal candidate will have an active California LVN license, current BLS/CPR certification, and experience in post-acute care, skilled nursing, or long-term care.
Requirements
- Active, unrestricted California Licensed Vocational Nurse (LVN) license.
- Current BLS/CPR certification.
- Strong communication, organization, and multitasking skills.
- Ability to work effectively within a multidisciplinary clinical team.
Nice To Haves
- Previous experience in post-acute care, skilled nursing, or long-term care preferred.
Responsibilities
- Provide direct, high-quality nursing care to residents according to individual care plans.
- Administer medications and treatments accurately and in a timely manner.
- Monitor, record, and report changes in resident conditions to physicians and families.
- Complete accurate and timely electronic documentation (EMR).
- Supervise and guide Certified Nursing Assistants (CNAs) on shift.
- Assist with resident admissions, discharges, and transfers.
