About The Position
ANX provides 24/7 hospice care for individuals with limited life expectancies or for patients no longer willing to seek aggressive medical treatment. Hospice care is provided by our specially-trained team of health care professionals, including a medical director/physician, medical social worker, skilled nurse, home health aide, spiritual counselor and hospice volunteer. ANX is committed to providing expert, reliable care utilizing best practices with an emphasis on community health.
Requirements
- Current CA Licensed Vocational Nurse (LVN) license
- Current IV Certification
- Current CPR card for Healthcare Providers
- 1 year of Licensed Vocational Nurse (LVN) experience
- Must be a licensed driver with an automobile that is insured in accordance with state and organization requirements and is in good working order.
- Demonstrates excellent verbal, written communication, and organization skills.
Responsibilities
- Provide direct patient care and treatment in a home setting under the supervision of a Registered Nurse.
- Ensure high quality patient experience and positive health outcomes by delivering the plan of care established by the RN Case Manager.
- Prepare complete and timely clinical notes, coordinate care delivery with an interdisciplinary team (PT, OT, ST, MSW, and HHA).
