About The Position

Felton Institute is seeking a Licensed Vocational Nurse (LVN) to join its Transitional Age Youth (TAY) and Adult Intensive Case Management Programs. Felton Institute is dedicated to providing innovative, evidence-informed social services that transform lives, with over 50 programs addressing mental health, homelessness, early care and education, the justice system, transitional age youth, and aging adults. Our programs integrate scientific research with cultural sensitivity and a commitment to serving our communities. Our mental health programs offer comprehensive, recovery-oriented services to transitional age youth (TAY) and adults (ages 16-60) with severe mental illness, often including co-occurring substance use disorders and complex physical health conditions. The multidisciplinary team provides psychiatric medical care, case management, therapy, and crisis intervention in clinic and community settings. Services assist clients in accessing housing, primary care, benefits, employment, substance use treatment, and other essential supports. This role is part of a collaborative team focused on promoting recovery, stability, and independence through person-centered, trauma-informed care. Community outreach is a key component, involving medication support, wellness checks, accompanying clients to medical appointments, and engaging individuals in their living spaces to ensure continuity of care.

Requirements

Completion of training in an accredited Licensed Vocational Nurse Program.
Active Licensed Vocational Nurse license from the California Board of Licensed Vocational Nurse.
Demonstrated commitment to the welfare of individuals and families with mental and behavioral health issues.
Knowledge of the information and techniques needed to diagnose and treat human injuries, diseases, and psychosis. This includes symptoms, treatment alternatives, drug properties and interactions, and preventative healthcare measures.
Complex problem solving, critical thinking, and social perceptiveness skills, i.e. being aware of others' reactions and understanding why they react as they do.
Service orientation skills; actively looking for ways to help people.
Demonstrated interest in ongoing learning and training.
Ability to be a team player with a welcoming attitude.
Willingness to travel for outreach to clients in the community.

Nice To Haves

Experience working with SMI (severely mentally ill) populations and community mental health settings preferred.

Responsibilities

Exercises medical responsibilities for psychiatric evaluation, medication assessment, and medication monitoring.
Works with an interdisciplinary team that includes clinical case managers, case managers, and other allied health professionals in the community.
Works in collaboration with the clinical team to provide therapeutic support and psychoeducation to clients, including but not limited to harm reduction, relapse prevention, and motivational interviewing.
Works in collaboration with the clinical team to make appropriate referrals and linkages to other services.
Coordination of care with outside providers to provide more comprehensive care.
Maintains clinical records in accordance with Medi-Cal standards and Felton policies.
Consults and provides on-going education to non-medical staff in psychopharmacology and medical aspects of patient care.
Performs additional duties as assigned by supervisor or assignee.
Exercises medical responsibilities for psychiatric evaluation and health assessment, medication administration including monitoring for side effects, monitoring for changes in health and mental health status, coordinating care with prescribers, making referrals for additional care, and pharmacy coordination and communication, as part of the treatment team.
Assists with crisis interventions, including 5150 evaluations, as needed, and communicates medical issues with Primary Health Care providers.
Maintains client caseload and clinical records in accordance with mandated standards.
Consults and provides on-going education to non-medical staff in psychopharmacology and medical aspects of patient care.
Community outreach is an integral part of the position, including providing medication support, conducting wellness checks, accompanying clients to medical appointments, and engaging individuals where they live to ensure continuity of care.
