Felton Institute is seeking a Licensed Vocational Nurse (LVN) to join its Transitional Age Youth (TAY) and Adult Intensive Case Management Programs. Felton Institute is dedicated to providing innovative, evidence-informed social services that transform lives, with over 50 programs addressing mental health, homelessness, early care and education, the justice system, transitional age youth, and aging adults. Our programs integrate scientific research with cultural sensitivity and a commitment to serving our communities. Our mental health programs offer comprehensive, recovery-oriented services to transitional age youth (TAY) and adults (ages 16-60) with severe mental illness, often including co-occurring substance use disorders and complex physical health conditions. The multidisciplinary team provides psychiatric medical care, case management, therapy, and crisis intervention in clinic and community settings. Services assist clients in accessing housing, primary care, benefits, employment, substance use treatment, and other essential supports. This role is part of a collaborative team focused on promoting recovery, stability, and independence through person-centered, trauma-informed care. Community outreach is a key component, involving medication support, wellness checks, accompanying clients to medical appointments, and engaging individuals in their living spaces to ensure continuity of care.
