About The Position
Anchor Co-Living is seeking dedicated Licensed Vocational Nurse (LVN) team members to join our Recuperative Care & Housing team. This role serves individuals recovering from illness, injury, or hospitalization who lack a safe place to heal. The position blends clinical care with compassion, helping residents stabilize medically while connecting them to housing, primary care, and community supports under California's CalAIM Community Supports initiative. The work is performed in a non-acute, residential setting with a small caseload and a supportive interdisciplinary team.
Requirements
- Active LVN license in the state of California (in good standing)
- Current CPR/BLS certification
- 2+ years of experience in direct patient care (home health, skilled nursing, or similar)
- Physically capable of assisting patients with mobility and transfers
- Positive attitude, emotional resilience, and team-first mindset
- Must have reliable transportation or commuting ability
Nice To Haves
- Experience with Alzheimer's, dementia, or chronic illness management a plus
Responsibilities
- Administer medications, perform wound care, and monitor vital signs
- Provide support for daily living activities with compassion and respect
- Identify changes in patient condition and report to supervising clinicians
- Maintain accurate, timely documentation in compliance with standards
- Promote a safe, clean, and positive care environment
