We’re growing and looking for an experienced Licensed Vocational Nurse (LVN) to join our Hospice Care team!
Requirements
- Active CA LVN License
- Minimum 1 year of LVN experience
- Current CPR certification
- Valid driver’s license and insured, reliable vehicle
- Ability to work independently and as part of a team
Nice To Haves
- Hospice experience preferred
Responsibilities
- Provide direct patient care under the supervision of a Registered Nurse (RN).
- Follow the plan of care.
- Deliver treatments.
- Collaborate with the interdisciplinary team to achieve positive patient outcomes.
- Implement nursing care based on comprehensive assessment and plan of care.
- Document patient services accurately and timely.
- Educate patients and families on diagnosis and self-care.
- Coordinate hospice services and report needs to the care team.
- Manage pain and symptoms; evaluate patient response to medications.
- Use equipment and supplies effectively.
- Participate in quality improvement and professional development.
