About The Position

Licensed Vocational Nurse (LVN) – Hospice Coverage Area: Alameda County Pay: $38.00 – $43.00 per hour Job Type: Full-Time, Part-Time, Per Diem The Sequoia Difference At Sequoia Hospice, we’re passionate about delivering exceptional, individualized care. We’re growing and looking for an experienced Licensed Vocational Nurse (LVN) to join our Hospice Care team!

Requirements

• Active CA LVN License
• Minimum 1 year of LVN experience
• Current CPR certification
• Valid driver’s license and insured, reliable vehicle
• Ability to work independently and as part of a team

Nice To Haves

• hospice experience preferred

Responsibilities

• Implement nursing care based on comprehensive assessment and plan of care
• Document patient services accurately and timely
• Educate patients and families on diagnosis and self-care
• Coordinate hospice services and report needs to the care team
• Manage pain and symptoms; evaluate patient response to medications
• Use equipment and supplies effectively
• Participate in quality improvement and professional development
