About The Position

The primary function of this role is to administer skilled nursing care, under the supervision of a registered nurse, to patients in their place of residence. The LVN will coordinate care with the interdisciplinary team and patient/caregivers. This position aims to promote patients' potential during their home health episode and collaborates with the physician, clinical team, and patient in preparation for discharge.

Requirements

  • Licensed Vocational Nurse, per the state requirements.
  • 1 year of Home Health experience.
  • Good communication skills.
  • Good assessment skills.
  • Strong clinical documentation skills.
  • Ability to read, write, and speak the English language.
  • Reliable transportation.
  • Valid and current auto liability insurance.

Responsibilities

  • Administer skilled nursing care under the supervision of a registered nurse.
  • Coordinate care with the interdisciplinary team and patient/caregivers.
  • Promote patients' potential during their home health episode.
  • Collaborate with physician, clinical team, and patient in preparation for discharge.
  • Identify patient’s physical, psychosocial, and environmental needs under the direction of the RN.
  • Participate in planning and implementing care in conjunction with the RN, in accordance with the POC.
  • Provide care utilizing infection control measures that protect both staff and patient (OSHA).
  • Record treatments administered in the medical record.
  • Record clinical documentation according to accepted regulatory, corporate and professional guidelines.
  • Participate in weekly case conferences.
  • Participate in interdisciplinary communication to ensure patients’ needs and expectations for services provided are understood and to ensure optimization of service delivery and patient progress.
  • Provide verbal or written patient status updates to supervising RN prior to reassessment, supervisory and/or discharge visits, and as indicated by patient response to treatment.
  • Participate in in-service training programs.
  • Assure patient safety.
  • Comply with state regulations and national standards.
  • Complete and sync all documentation by 9:00 AM the following day, 7 days per week.
