We are currently seeking a compassionate, organized, and team-oriented individual to join our Home Health team as a Full-Time Licensed Vocational Nurse (LVN) in Oakland, CA. We are seeking an experienced, self-driven Full-Time Licensed Vocational Nurse (LVN) to join our Home Health team and provide exceptional patient care in the region. As an LVN, you will assist in implementing individualized care plans, monitor patient progress, and provide critical support in the field. You’ll work closely with our Clinical Director, Registered Nurses, and Clinical Staff, ensuring high-quality patient care and support.

Requirements

  • Current Licensed Vocational Nurse (LVN) License to practice in California - Required
  • Intravenous Therapy (IV) Certified and Wound Care / Wound Nursing experience - Required
  • Experience in Home Health or Hospice care - 1 Year Required
  • Demonstrate compliance with accepted professional standards and practices.
  • Strong verbal and written communication skills, with excellent organizational skills.
  • Current CA Driver’s License
  • Able to travel within a 10 - 15 mile radius on a regular basis.

Responsibilities

  • Implement the plan of care developed by the Registered Nurse and provide treatment in accordance with patient needs.
  • Accurately document patient care in a timely manner, consistent with the plan of care.
  • Assess and educate patients and family members/caregivers on diagnosis and care plans.
  • Coordinate with other home health services (such as Physical Therapy, Occupational Therapy, Speech Therapy, or Social Work) as needed and report these needs to the RN or Clinical Supervisor.
  • Use equipment and supplies effectively and efficiently, ensuring all care adheres to company policies.
  • Maintain accurate clinical records and submit timely documentation via online charting in Kinnser, using company-provided devices.
  • Follow all company policies and procedures to ensure high standards of care.
