Requirements

  • Graduate of an accredited Certificate, Diploma, or Associate Degree Nursing program
  • Current Texas Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) license
  • Minimum of one (1) year of nursing experience in a clinical setting
  • Excellent communication and organizational skills
  • Valid driver's license, reliable transportation, and current automobile insurance
  • Ability and willingness to travel throughout the assigned service area

Responsibilities

  • Provide skilled nursing visits according to the physician's orders and individualized plan of care.
  • Monitor patient conditions and promptly report significant changes to the RN Case Manager and physician.
  • Assist patients and caregivers with education on medications, treatments, disease management, and self-care.
  • Administer treatments and nursing interventions within your scope of practice.
  • Collaborate closely with the RN Case Manager and interdisciplinary care team.
  • Document all patient care accurately and in a timely manner.
  • Identify safety concerns and help implement interventions that reduce hospitalizations and emergency room visits.
  • Deliver compassionate, patient-centered care while maintaining compliance with all clinical and regulatory standards.
