About The Position

Coordinates resident nursing care on the SNF/CDP Unit. Responsibilities include maintaining current professional licensure and CPR certification, completing required competencies and health and safety requirements, orienting new staff, and upholding residents' rights. The role involves observing residents, reporting elder abuse, and providing care sensitive to age, culture, religion, gender, and sexual orientation. It also includes controlling the environment to prevent hazards and cross-infection, attempting alternatives to restraints, and communicating effectively with residents, families, and healthcare team members. The LVN assesses residents daily, communicates concerns to the Charge RN and Clinical Director, and uses supplies and resources appropriately to ensure financial soundness of the department.

Requirements

  • Current Licensed Vocational Nurse (LVN)
  • BLS Certification

  • IV/Phlebotomy certification is preferred

Responsibilities

  • Maintains current Professional License (LVN) and appropriate CPR Certification and provides facility with a copy of these documents in a timely manner to ensure practice is covered by active License and Certification at all times.
  • Keeps up-to-date educationally in areas of Specialty.
  • Completes initial and annual competencies and annual Health and Safety requirements timely.
  • Orients new LVN’s as assigned.
  • Assists in appropriately orienting registry and float staff as assigned by Charge RN.
  • Is helpful, positive and supportive when working with registry and float staff.
  • Knowledge of Residents Bill of Rights and Elder Abuse reporting regulations.
  • Observes Residents Bill of Rights and reports incidents of elder abuse, if indicated.
  • Carries out duties in a manner sensitive to and appropriate to age (primarily adults and older adults), culture, religion, gender, and sexual orientation of the residents.
  • Control the environment to prevent hazards and cross infection by observing HDMHD policies and procedures.
  • Attempts with guidance from the Charge RN, alternatives to physical and chemical restraints are attempted before requesting use of physical and chemical restraints.
  • Is familiar with SDS information sources on the Unit.
  • Communicates with Charge RN, other team leaders and CNA staff, residents, family members and visitors, to ensure a professional two-way flow of information regarding resident care and other issues.
  • Assesses residents daily and on an as needed basis, and communicate concerns to the Charge RN using sound nursing judgment and decision making skills.
  • Communicates regularly with the Clinical Director regarding resident care and other issues not resolved by discussion with the Charge RN.
  • Helps ensure department operates in a financially sound manner by using supplies and resources appropriately.
