About The Position

The Licensed Vocational Nurse (LVN) provides direct general medical and psychiatric nursing care to residents. This includes completing all facility-designated assessment instruments, collecting and documenting assessment data, developing and updating resident care plans, implementing physician orders and care plans, administering medications, performing prescribed treatments, monitoring residents, documenting care, and communicating changes in condition. The LVN practices under the direction of a physician or Registered Nurse, with nursing oversight from the Director of Nursing and administrative supervision from the Administrator. The role involves collaboration with a multidisciplinary team, including physicians, Director of Nursing, Administrator, pharmacy personnel, case managers, therapists, support staff, Mental Health Workers, and Activities staff. The LVN may also perform duties of a Certified Nurse Assistant, support staff, Mental Health Worker, or Activities staff member when trained and competent, provided these assignments do not interfere with core licensed nursing responsibilities.

Requirements

  • Current, active, and unrestricted California Licensed Vocational Nurse license issued by the Board of Vocational Nursing and Psychiatric Technicians.
  • Graduation from a California-approved vocational nursing program or other educational pathway accepted by the Board for California LVN licensure.
  • Current Healthcare Provider Basic Life Support certification.
  • Ability to pass required background, health-screening, tuberculosis, and employment-clearance requirements.
  • Ability to use PointClickCare or the designated electronic health record, eMAR, secure communication systems, and standard office technology.
  • Ability to communicate effectively in writing and verbally with residents, physicians, staff, families, case managers, and other authorized parties.
  • Maintains eligibility to work in the licensed facility setting and immediately discloses to the Director of Nursing, Administrator, and Human Resources any license lapse, restriction, accusation, disciplinary action, exclusion, criminal matter, or other event that may affect the employee's authority or eligibility to perform assigned duties.
  • Completes initial and ongoing competency validation for assigned clinical procedures, assessment instruments, medication systems, emergency procedures, and Lead responsibilities, as applicable to the assigned classification.

  • IV therapy and blood-withdrawal certification when required by assigned duties; otherwise preferred but not required.

  • Maintains eligibility to work in the licensed facility setting.
  • Discloses any license lapse, restriction, or disciplinary action.
  • Completes initial and ongoing competency validation for assigned clinical procedures.
