About The Position

The County of Riverside - Riverside University Health System (RUHS) - Community Health Centers is seeking a bilingual Licensed Vocational Nurse II to join their MSC-Internal Medicine team. Under supervision and direction of a registered nurse, nurse practitioner, physician assistant, or physician, performs nursing duties within the scope of the Nurse Practice Act for vocational nursing in the care of patients in the Department of Mental Health or Public Health Ambulatory Family Health Center such as thorough patient intake including vital signs, addressing care gaps, and CGM downloads, obtaining STAT referral authorizations for specialty appointments and DME, scheduling and rescheduling patients, completing e-faxes and incoming clinic mail, making daily warm reminder calls, assisting with supply inventory, managing in-basket requests, CRMs, and patient messages, participating in Rapid Improvement Events and process improvement projects, supporting resident care team assignments, completing SPEC notes, administering vaccines and medications, preparing for EKGs, cervical cancer screenings, and other clinic procedures, providing discharge instructions and patient education, processing refill requests, completing Chase Lists, serving as an MA preceptor for vaccine sign-off, in-basket training, and point-of-care testing, performing point-of-care testing for clinic patients, assisting with MyChart activation, maintaining clinic inventory, supporting CODE MET/triage nurse duties, performing resource nurse responsibilities, floating to other specialties or sister clinics as needed, assisting with late-stay internal medicine schedules, championing metric goals for access, quality, and performance improvement, and performing all other duties within the LVN scope of practice ; and perform other related duties as required. The department is seeking a high energy, detail oriented LVN with strong clinical and critical thinking skills to join their high-volume clinic with complex and chronic condition patients. Internal medicine staff supports 8 attendings and 64 residents, who rotate every 6 weeks. LVNs in this department also support our specialty clinics as needed: Cardiology, Rheumatology, Infectious disease, and Endocrinology as needed. Additionally, LVNs float to other sister clinics within the MSC and other CHCs if support is needed. This role requires rapid critical thinking, swift multi-tasking and exceptional triage capabilities. The incumbent must be detail-oriented and able to lead a team of medical assistants. The most competitive candidates will have a minimum of 2 years of LVN experience in a high pace clinic, urgent care, or hospital setting.

Requirements

  • California Vocational Nurse's License issued by the California Board of Vocational Nurse and Psychiatric Technician Examiners
  • Basic Life Support (BLS) certification issued by the American Heart Association
  • IV Therapy/Blood Withdrawal certified by the Board of CA Vocational Nursing and Psychiatric Technicians
  • Advanced Cardiovascular Life Support (ACLS)
  • At least one year of successful experience as an LVN, preferably in a public health clinic environment.
  • Knowledge of current principles, techniques and procedures used in vocational nursing within the scope of the Nurse Practice Act for vocational nursing.
  • Knowledge of medical terminology, acute care procedures and equipment.
  • Knowledge of medication administration and any policies and procedures of the setting assigned.
  • Ability to skillfully perform the full range of vocational nursing duties and responsibilities within the scope of the Nurse Practice Act for vocational nursing.
  • Ability to learn and follow policies and procedures for the setting assigned.
  • Ability to establish and maintain effective working relationships with physicians, mid-level providers, registered nurses, patients, and co-workers.
  • Ability to complete and maintain reports and patient records.
  • Ability to make observations of situations accurately and report them so effective action can be taken according to rules of documentation and policies and procedures.
  • Possession of a valid California Vocational Nurse's License issued by the California Board of Vocational Nurse and Psychiatric Technician Examiners.
  • Possession of a valid Basic Life Support (BLS) certification issued by the American Heart Association for professional healthcare providers and IV Therapy/Blood Withdrawal certified by the Board of CA Vocational Nursing and Psychiatric Technicians, and Advanced Cardiovascular Life Support (ACLS).
  • A valid California Driver's License is required, as travel may be required in the event the LVN is floated to sister CHC facilities.
  • Employees in this classification are required to be able to lift up to 50 lbs. unassisted with or without reasonable accommodation, push patients in wheelchairs, and walk/stand/sit for long periods.

Responsibilities

  • Perform nursing duties within the scope of the Nurse Practice Act for vocational nursing in the care of patients.

  • Apply and change dressings and care for wounds.
  • Collect specimens for laboratory analysis.
  • Assist physicians, registered nurses, and mid-level providers in performing examinations, tests, respiratory care and other treatments including minor surgical procedures, PAP smears and colposcopies.
  • Perform venipuncture, injections of medications including biological immunizations for pediatrics and adults, including the provision of Tuberculin skin tests, Coccidioidin skin tests, and Histoplasmin skin tests in the course of a tuberculosis control program.
  • Perform documentation as required within the scope of the Nurse Practice Act for vocational nursing, and established rules of charting.
  • Chart treatments and medications and maintain pertinent medical records.
  • Collect data from patients to ensure scheduling with appropriate services.
  • Maintain clinical records, reports and charge tickets.
  • Review charts to determine type of treatment the patient may require and assure proper clinic scheduling and continuity of care.
  • Counsel, advise and educate patients regarding proper health care methods.
  • Set clinic appointments per established policies and procedures.
  • Refer patients for specialty care to outside agencies utilizing knowledge of payor source to ensure appropriate referrals to outside agencies and programs.
  • Assist physicians, registered nurses, and mid-level providers in the administration of special treatments, examinations, and intravenous therapy.
  • Chart treatments and medications and maintain pertinent medical records according to established practice by the Nurse Practice Act for vocational nursing, policies, and procedures.

  • Assist in the data collection, preparation, and implementation of patient care plans.
  • Participate in patient education and rehabilitation programs and document data appropriately.

  • Act under supervision and direction of a physician, registered nurse or mid-level provider as a nursing leader over health services assistants, certified medical assistants, office assistants and other LVNs, as assigned.
