About The Position

In partnership with the LA County Department of Mental Health, UMMA Health’s Integrated Care Program helps communities access both traditional and non-traditional care. Under the supervision of the Integrated Care Program Manager, the Chronic Care Case Manager/LVN will provide coordination and tracking of medical services to enrolled clients (.50 FTE) as well as outreach, engagement and education to enrolled clients and in the community (.50 FTE).

Requirements

  • Graduate from an approved nursing program with degree, or equivalent degree
  • California License as a Licensed Vocational Nurse or interim permit for LVN
  • Current CPR/BLS Certification
  • Must have knowledge through practical experience and/or training in the areas of: Public Health Nursing, Clinic Nursing
  • Must be aggressive in developing of patient care programs within the clinical policies and programs objective and relating them to the funding requirements
  • Must be sensitive to the needs of the patients in a community health care center setting, in the areas of health and social services as one component
  • Excellent knowledge of case management principles and healthcare management.
  • Ability to work with patients with disabilities, mental illnesses, or challenging behaviors.
  • Good communication and interpersonal skills.
  • Ability to be compassionate, empathetic, patient, and calm when working with patients.
  • Solid organizational skills and a high level of attention to detail.
  • Ability to remain up-to-date with emerging medical knowledge and technology.

Responsibilities

  • Evaluating patients' treatment plans
  • Monitoring patient well-being
  • Creating case progress reports
  • Ensuring patients are well informed regarding their care and treatment options
  • Provide diverse, complex and confidential clinical coordination of ISM Enrolled Clients.
  • Provide individual and group health education on chronic medical conditions to enrolled clients and in the community.
  • Participate in, and provide medical updates during weekly case conferences.
  • Identify and submit needed medical supplies to support work with clients and outreach and engagement activities.
  • Identify and implement outreach, engagement and education activities to recruit clients to program.
  • Provide monthly reports on case management services and outreach and education activities.
  • Collect, track and input medical outcome data in EHR system
  • Demonstrate patterns of continued learning in order to ensure growth as a practitioner.
  • Participate in Quality Improvement as required.
  • Perform other duties as required by supervisors (and providers when appropriate).
  • Perform basic clinical related duties as necessary.
