About The Position

Billet Health is seeking compassionate and dedicated Licensed Practical Nurses (LPNs) to provide exceptional patient care within the comfort of patients' homes and residential facilities. As part of the Home Health team, you will collaborate with the RN Case Manager and Director of Nursing to assess patient needs, deliver quality care, and ensure patients receive the attention and support they deserve. The role offers flexible scheduling, allowing nurses to determine their workload, from seeing several patients daily to working a few days a week or building a full-time schedule. Patient visits are typically 45-60 minutes and are coordinated directly with patients or caregivers.

Requirements

• Active LPN license in the state of Arizona
• Valid driver's license
• Reliable transportation
• Current auto insurance
• CPR or BLS certification
• Valid TB documentation (Skin test within the last year, or Chest X-ray within the last five years)

Responsibilities

• Providing skilled nursing care in patients' homes or residential facilities
• Assessing patient needs and reporting changes in condition
• Following the established plan of care
• Working closely with the RN Case Manager and Director of Nursing
• Providing patient and caregiver education
• Monitoring and documenting patient progress
• Communicating with physicians and members of the interdisciplinary care team
• Promoting patient safety, comfort, and independence
• Completing accurate and timely clinical documentation
