About The Position

A Place of Grace, Inc. is seeking a knowledgeable, organized, and dependable Licensed Vocational Nurse Consultant to support the health care coordination and medication-management needs of residents at their Vermel facility, a CPP-designated Enhanced Behavioral Supports Home (EBSH). The LVN Consultant will work collaboratively with the Administrators and Registered Nurse Consultant to help ensure medications remain available and resident appointments, referrals, health care recommendations, and required follow-up care are properly tracked and completed. This is a consulting position with variable hours, where the amount of clinical support required for each resident may change based on individual health care needs, appointments, referrals, medication needs, and necessary follow-up. All duties must be performed within the LVN’s authorized scope of practice and under RN oversight when required.

Requirements

  • Current and valid California Licensed Vocational Nurse license issued by the California Board of Vocational Nursing and Psychiatric Technicians.
  • At least one year of experience working with individuals with intellectual or developmental disabilities or a similar population.
  • Knowledge of medication management, health-related documentation, appointment coordination, and the LVN scope of practice.
  • Strong organizational skills and the ability to manage multiple appointments, referrals, medication needs, and follow-up requirements.
  • Strong written and verbal communication skills.
  • Ability to maintain accurate and confidential resident records.
  • Ability to identify concerns requiring escalation to the Administrator, RN Consultant, or appropriate health care provider.
  • Ability to work collaboratively with administrators, direct support staff, health care providers, pharmacies, and other members of the residents’ support teams.

Responsibilities

  • Coordinate and track medical, dental, psychiatric, laboratory, referral, and follow-up appointments, including transportation needs and required documentation.
  • Obtain, organize, and review physician orders, prescriptions, After Visit Summaries, discharge instructions, laboratory reports, and health care recommendations.
  • Ensure routine and time-sensitive follow-up items are documented and communicated to the Administrator, RN Consultant, and designated staff.
  • Oversee routine medication-management activities, including Medication Administration Records, medication orders, medication availability, supplies, refills, pharmacy deliveries, discontinued or duplicate medications, and medication disposal records.
  • Follow up on unavailable medications, medication errors, missed doses, refusals, adverse reactions, and documentation concerns.
  • Communicate with pharmacies, health care providers, the Administrator, and the RN Consultant to resolve medication and health care coordination issues.
  • Maintain current, accurate, confidential, and organized health care, appointment, medication, and physician-order records.
  • Assist with health-related reports, new-resident coordination, and Individual Behavior Support Team meetings as requested.
  • Provide staff instruction and support regarding medication assistance and documentation, general health practices, infection control, and the recognition and reporting of health concerns within the LVN’s scope of practice.
  • Promptly notify and consult with the Administrator and RN Consultant regarding significant changes in a resident’s condition, adverse reactions, medication concerns, complex medical needs, or matters requiring additional clinical assessment or judgment.
  • Perform other duties consistent with the LVN’s license, authorized scope of practice, and assigned responsibilities.
