About The Position

Family Health Centers of San Diego (FHCSD) is seeking a Licensed Therapist (LCSW, LMFT, LPCC, or LCP) to join their growing Mental Health Department. FHCSD is dedicated to providing exceptional health care to all, especially underserved communities. With a history spanning over 50 years, FHCSD has grown into one of the largest community health systems in the country, operating over 90 sites and serving a diverse patient population. The Mental Health Department aims to create an inclusive community that fosters teamwork and continuous learning, offering support through a structured orientation process and a Buddy Program for new therapists. Services are outpatient and designed for individuals with mild to moderate acuity levels.

Requirements

• Must have an active registration in good standing with the State of California as a Licensed Marriage and Family Therapist (LMFT), Licensed Clinical Social Worker (LCSW), Licensed Professional Clinical Counselors (LPCC) or Clinical Psychologist.

Responsibilities

• Provide counseling sessions to assigned patients
• Complete all required documentation accurately and timely
• Performs initial assessment and on-going care
• Participates in the development of individualized care plans
• Addresses co-occurring disorders
• Work as a member of the multi-disciplinary treatment team
• Coordinate with Case Managers and patient support staff
• Connect patients to internal/external resources as appropriate
• Performs other duties as assigned
