Licensed Therapist, LCSW: Palliative Care
About The Position
The Therapist provides comprehensive assessments, coordination, planning, and implementation of a psychosocial treatment plan for patients in respective departments. This role promotes optimal mental health by providing ongoing psychosocial support and services to patients and their families for adjustments to illness, grief, addiction, and other psychosocial stressors. The Therapist works with an interdisciplinary team to achieve treatment goals, utilizing high-risk criteria, medical records, school records, clients, families, nurses, physicians, and others to evaluate patient’s biopsychosocial needs. A treatment plan and a comprehensive termination of treatment/discharge plan are developed. Objective and accurate data regarding assessments and interventions are documented. The Therapist maintains working knowledge of community-based resources, programs, and social services, and educates patients and families on their health care options, connecting them with appropriate resources. The role may involve developing and facilitating support groups and workshops, providing input on treatment schedules, patient assignments, and crisis interventions as requested by the Clinical Manager, and performing other duties as assigned. Additionally, the Licensed Therapist may provide ongoing teaching and supervision to MSW employees and interns and may sign off on State requirements for LCSW licensing. Advanced clinical knowledge is utilized to assess and identify psychiatric and psychosocial issues facing patients. Assessments related to legal ethical reporting requirements may be conducted. Some departments provide additional specialist services such as Reiki Therapy when the clinician is certified to do so.
