About The Position

The Intake Coordinator serves as the first point of contact for individuals seeking mental health services and plays a vital role in connecting them with appropriate care. This licensed mental health professional manages the intake and referral process from initial inquiry through admission or referral to community resources, ensuring timely access to services. The Intake Coordinator reviews clinical and eligibility information, collaborates with multidisciplinary teams, maintains accurate documentation, and supports a positive, person-centered experience while upholding confidentiality and regulatory standards.

Requirements

  • Active North Carolina licensure as a mental health professional required (LCSW/LCSW-A, LCMHC/LCMHC-A, LMFT, Licensed Psychologist, Licensed Psychological Associate, or Licensed Psychiatric Nurse Practitioner).
  • Minimum of two years of experience working with adults diagnosed with Severe and Persistent Mental Illness (SPMI).
  • Experience conducting comprehensive clinical assessments and participating in intake and referral processes.
  • Knowledge of community-based behavioral health services, recovery-oriented care, and the needs of individuals with SPMI.
  • Strong clinical judgment with the ability to identify and respond to urgent behavioral health concerns.
  • Excellent communication, interpersonal, and customer service skills.
  • Ability to build rapport with clients, families, referral sources, and community partners.
  • Strong organizational skills with the ability to manage multiple priorities and meet deadlines.
  • Proficiency with electronic health records (EHRs) and documentation systems.
  • Ability to maintain confidentiality and comply with applicable regulations, policies, and ethical standards.
  • Ability to work collaboratively as part of an interdisciplinary treatment team.

Nice To Haves

  • Experience with ACTT, FACT, or other community-based behavioral health programs preferred.
  • Leadership, management, or clinical supervisory experience preferred.
  • Valid driver's license and reliable transportation may be required based on program needs.

Responsibilities

  • Receive, review, and track referrals from individuals, families, providers, hospitals, and community partners.
  • Gather demographic, clinical, and referral information and obtain required documentation.
  • Conduct initial clinical screenings and identify urgent concerns requiring clinical intervention.
  • Coordinate eligibility reviews, program fit assessments, and admission decisions with clinical leadership.
  • Complete Comprehensive Assessments for individuals referred to the ACT Team (ACTT).
  • Schedule intake assessments and ensure all required consents and paperwork are completed.
  • Maintain accurate referral logs, intake records, and documentation in designated systems.
  • Communicate professionally with members, families, referral sources, and treatment staff.
  • Facilitate a smooth transition into services and provide community resource referrals when appropriate.
  • Collaborate with Team Leads, the Clinical Director, and interdisciplinary team members to support quality care.
  • Participate in meetings, training, quality improvement initiatives, and other duties as assigned.
