About The Position

Easterseals PORT Health (ESPH) is seeking a Licensed Substance Abuse Counselor to join our Assertive Community Treatment (ACT) Team. This role involves providing life-changing care directly in the community to help clients on their path to recovery from mental health and substance use challenges. The ACT Team delivers care in clients' homes, shelters, and on the streets, offering personalized care and guiding clients toward lasting recovery. This position offers the chance to deliver vital substance use interventions, collaborate with a skilled multidisciplinary team, and advocate for clients' needs.

Requirements

Certified as a CADC or CCS, or provisionally or fully licensed as a LCAS/A in NC.
Comfortable working in community settings.
Valid driver’s license.

Responsibilities

Conduct substance use assessments and provide individual and group interventions.
Facilitate access to recovery resources, including 12-step programs.
Collaborate with the ACT team to develop tailored treatment plans.
Participate in daily team meetings to ensure quality service delivery.
Maintain accurate and timely documentation using Welligent EMR.
Offer on-call crisis intervention services as part of a rotating schedule.
