About The Position

This position conducts psychosocial assessments with clients and/or significant others to determine and evaluate various factors including support systems, emotional status, substance use, communication barriers, living situations, education, financial stress, need for community resources, and personal safety. The role involves proactive patient engagement for those screening positive for social determinants of health, connecting them to community resources, and making appropriate referrals. The social worker will collaborate with multidisciplinary teams, other medical providers, clinicians, and community partners to address client needs. Responsibilities include participating in case conferences, providing counseling and social psychotherapy, facilitating referrals, and documenting treatment objectives and results. The role also involves coordinating financial assistance referrals, ensuring accurate clinical documentation, managing a complex caseload, maintaining confidentiality, and promoting customer service. Other job-related duties may be assigned.

Requirements

  • Master’s Degree in Social Work
  • Exceptional problem-solving, critical thinking, organizational, interpersonal, and written/verbal communication skills.
  • Ability to work in a self-directed environment with attention to detail and follow-through.
  • High level of computer literacy.
  • History of positive rapport with patients, families, physicians, and interdisciplinary team.
  • Active LISW license

Nice To Haves

  • Knowledge of community resources and referrals in all service areas.
  • Must be able to use computerized documentation system and laptop computer.
  • Successful completion of a clinical social work field practicum from a CSWE accredited program.
  • One or more years of social work experience in a medical or behavioral Health Care setting.
  • LSW in the state of Michigan.
  • LMSW or LLMSW in the state of Michigan.

Responsibilities

  • Conducts psychosocial assessments with clients and/or significant others to determine and evaluate support systems, family relationships, emotional/psychological/cognitive status, substance use/abuse, communication/language barriers, living situation, education/vocational status, financial stress, need for community resources, social/emotional factors impacting treatment and recovery, and personal safety.
  • Conducts proactive patient engagement for patients who screen positive for social determinants of health and connects clients to available community resources.
  • Maintains knowledge of age-specific community resources for all geographic areas of service and makes referrals to appropriate agencies.
  • Works cooperatively with multidisciplinary teams, other medical providers, clinicians, case managers, and community partners to address and resolve client needs.
  • Participates in case conferences to identify social problems, their severity, and impact on patient health, as well as patient and caregiver strengths and weaknesses.
  • Provides counseling and social psychotherapy to clients and significant others regarding medical, behavioral, vocational, and social needs, and facilitates referrals for ongoing counseling/therapy.
  • Coordinates financial assistance referrals and refers clients/families for financial and employment coaching.
  • Ensures accuracy of clinical documentation, including assessments, re-visits, follow-up interventions, and resource connection tracking.
  • Demonstrates the ability to prioritize a complex caseload to effectively meet client and staff needs and maintain productivity expectations.
  • Maintains confidentiality of all agency and patient records.
  • Promotes and maintains customer service-focused care consistent with system values.
  • Performs other job-related duties as assigned.
