This position handles pressure, objectivity, confidentiality, organization, planning, reporting skills, persistence, proactivity, listening, verbal communication, and resident and family relationships. The role involves establishing a course of action by exploring options and setting goals with residents and their families. The Licensed Social Worker will assist residents in achieving the highest practicable level of self-care, independence, and well-being by providing medically related social services. This includes evaluating social and family information, psychological and emotional needs, and developing care plans. The role also involves providing emotional support, addressing emotional problems, assisting residents and families with coping mechanisms for physical disabilities, health-related fears, and grief. Additionally, the position supports residents with healthcare decisions, personal shopping, and inventory of patient belongings. The Licensed Social Worker will coordinate responses to reports of missing, lost, or stolen belongings, and assist in obtaining resources from community and social services agencies. They will also help secure appropriate prosthetics and assistive devices, develop strong working relationships with other departments and outside agencies, and document resident social service status. The role includes assisting with outpatient appointments and on-site ancillary patient services, as well as discharge planning with appropriate agencies and coordinating with the interdisciplinary team. In-service training to staff may also be requested, along with counseling residents and family members.
