About The Position
Park Manor of Conroe is seeking an experienced and dedicated Licensed Social Worker for their Skilled Nursing/Long Term Care facility. The Social Worker will assist in planning, organizing, implementing, evaluating, and directing the overall operation of the Social Service Department. The role involves providing medically related social work services to help each resident attain or maintain the highest practicable level of physical, mental, and psychosocial well-being.
Requirements
- Must have experience with PointClickCare
- 2 Years Social Work experience in a Long Term Care or SNF
- 4 year Degree
- Licensed Social Worker in the State of Texas
- Social Worker License
