About The Position

This role involves providing therapeutic services to patients, addressing referrals from various providers, and coordinating services. The position requires completing group and individual therapy sessions, maintaining scheduled appointments in electronic records, and documenting services using Bestnotes. Collaboration with a multidisciplinary team and external partners is essential. The role also includes completing necessary documentation such as biopsychosocial assessments and treatment plans, demonstrating cultural competency, and adhering to the agency's philosophy. A caseload of 8-10 patients weekly is expected for individual therapy sessions.

Requirements

• Must be a Licensed Social Worker
• Proficiency in using electronic records systems
• Experience with Bestnotes documentation
• Ability to conduct biopsychosocial assessments and treatment planning
• Demonstrated cultural competency
• Commitment to agency philosophy
• Ability to manage a caseload of 8-10 patients weekly
• Willingness to provide services in person or remotely

Responsibilities

• Addresses referrals from providers including referrals via warm hand-offs
• Complete an average of 4 group therapy sessions per week (8AM-10AM Monday through Thursday with occasional pick up groups)
• Participates to assess needs and coordinate services
• Services may be done in person or remote, but may be required to do services in person on occasion
• Maintains weekly scheduled appointments in our electronic records systems
• Use Bestnotes to document services in a timely manner
• Provide therapeutic services to patients
• Work in collaboration with multidisciplinary team members including external partners
• Completes required documentation and paperwork including biopsychosocial assessments and treatment planning and implementation in a timely manner
• Demonstrates cultural competency and commitment to patients
• Demonstrates commitment to the philosophy of the agency
• Maintain caseload of 8-10 patients weekly (for one hour individual therapy sessions, may be done virtually)
• Other duties as assigned
