About The Position

The Licensed Social Worker will perform social work evaluations of patient/family psychosocial and environmental needs. They will implement the social work treatment plan and document according to insurance guidelines in homes or facilities. This role assists the primary nurse in establishing and maintaining a treatment plan for each patient/family, focusing on the psychosocial needs of the hospice patient and their family. The position also involves assessing the needs of bereaved clients through interviews, providing end-of-life bereavement supportive services, and discussing available community resources for referrals. Additionally, the role includes participating in community education on grief and bereavement, and providing support to selected patients post-discharge. The Licensed Social Worker will represent the philosophy and services of Hospice of Huntington, Inc.

Requirements

  • MSW or BSW with appropriate proof of licensure/certification.
  • Minimum 1 year experience in a health care setting.
  • Licensed in WV and Ohio.
  • Demonstrate evidence of knowledge in understanding grief, bereavement, interpersonal issues and family dynamics.
  • Valid driver's license along with an insured, reliable vehicle required.
  • Employment is contingent upon successful completion of a background check, drug screening, and possession of a valid driver's license.

Nice To Haves

  • Minimum of one year experience in health care, with a preference in Hospice/Home Health Care.
  • Evidence of clinical experience and skills pertinent to assessing and counseling individuals.
