About The Position

Parker Jewish Institute for Health Care and Rehabilitation is seeking a Licensed Social Worker to provide social work services to assigned patients, residents, family members, and significant others in its nursing home. The role coordinates discharge planning, participates in interdisciplinary team meetings and quality improvement activities, and provides counseling while supporting patient, resident, and family satisfaction. The position reports to the Director of Social Work. The schedule is part-time: Mondays, Wednesdays, and Fridays from 9:00 a.m. to 5:00 p.m. Parker is a nonprofit health care facility located in New Hyde Park, New York, offering rehabilitation, long-term care, nursing home care, and community health services. Requirements

Master's Degree in Social Work.
Active licensure in New York State.
Strong communication skills.
Ability to work collaboratively across a variety of disciplines.
Familiarity with electronic medical records (EMR).

Nice To Haves

Prior experience in a long-term care, rehabilitation, or hospital setting.
Prior experience working with the elderly population.
