About The Position

Lutheran Senior LIFE is seeking a full-time Social Worker to join their team. This position will be based in Jersey City and will also involve visiting participants in their homes throughout Hudson County. The role focuses on conducting initial in-person assessments for potential participants to gather a comprehensive psycho-social history, including cognitive status, social supports, family dynamics, mental health, substance dependency, and other current issues. The Social Worker will collaborate with an interdisciplinary team to create individualized care plans for each participant. Requirements

  • Member of the Academy of Certified Social Workers (ACSW)
  • Current driver’s license and proof of auto insurance
  • Licensed with the New Jersey Board of Social Work
  • Experience with frail/chronically ill elderly people
  • MUST be English/Spanish bilingual
  • Ability to provide psychosocial assessment and individual, family and group counseling
  • Ability to maintain accurate records and to prepare clear and concise reports, correspondence and other written materials.
  • Good public speaking skills with all size groups
  • Verbal and listening skills; ability to speak in front of large audiences
  • Effective verbal and written communication skills
  • Demonstrated ability to work in an interdisciplinary team setting.
  • Minimum one year working with the frail or elderly.
  • Employees must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.

Nice To Haves

  • 2 years experience working on an interdisciplinary team in a hospital, nursing home or community-based setting is preferable.

Responsibilities

  • Performs in person initial assessments for enrollment of potential Lutheran Senior Healthcare participants to obtain a complete psycho-social history, which may include descriptions of cognitive status, social supports, family dynamics, mental health and substance dependency, and other current issues and needs.
  • Coordinates with the interdisciplinary team to develop a comprehensive care plan for each participant.
