About The Position

PrimeCare Medical is seeking a Licensed Professional Counselor (LPC) or Licensed Social Worker (LSW or LCSW) to join the medical department as a Behavioral Health Professional at the Montgomery County Correctional Facility in Eagleville, PA. This is a 40-hour position, Monday to Friday, during normal business hours (8:00 am - 4:30 pm). Occasional Saturday shifts or evening weekday coverage may be required with advance notice.

Requirements

  • Must be licensed as a LPC, LSW, or LCSW in the Commonwealth of Pennsylvania.
  • Must hold a Master’s degree or greater in Social Work, Psychology, or related Human Service field with a concentration in clinical coursework, training and/or experience.
  • Minimum of two years of experience.

Responsibilities

  • Conduct clinical assessments based on referrals and self-referrals, including documentation of findings, mental status examination, case conceptualization, diagnosis, and treatment plan. Refer to psychiatry when appropriate.
  • Conduct risk assessments using structured interviews and risk assessment tools. Manage inmates at risk for suicide and/or self-harm according to existing protocols and policies.
  • Serve as the PrimeCare Medical (PCM) mental health liaison for mental health services outside the facility.
  • Facilitate admission and placement of inmates/patients to forensic hospitals when necessary.
  • Monitor all mental health services provided.
  • Monitor clinical services to ensure standards of care align with NCCHC and accepted psychiatric/psychological standards.
  • Participate as a member of the Quality Assurance Committee.
  • Develop and conduct in-service education programs according to the annual in-service calendar.
  • Collect data and prepare monthly performance reports.
