About The Position

Professional Care Medical Practice P.C. is seeking a Licensed Mental Health Counselor (LMHC) or Licensed Social Worker (LMSW or LCSW) to serve as a Behavioral Health Professional within the medical department at the Onondaga County Justice Center in Syracuse, NY. This is a full-time, 40-hour per week position, with shifts from Tuesday to Saturday, 1:30 PM to 10:00 PM. The role involves providing comprehensive mental health care, including substance abuse and MAT counseling, to a vulnerable and underserved patient population within a correctional environment. Requirements

• Must be licensed as a LMHC, LMSW, or LCSW in the state of New York.
• Must hold a Master’s degree or greater in Social Work, Psychology, or related Human Service field with a concentration in clinical coursework, training and/or experience.
• Must hold a valid driver’s license & have reliable transportation.

Nice To Haves

• Minimum of two years of experience and familiarity with substance abuse/MAT (i.e., Medicated Assisted Treatment) counseling preferred.

Responsibilities

• Conduct clinical assessments based on referrals and self-referrals, including documentation of findings, mental status examination, case conceptualization, diagnosis, and treatment plan.
• Make referrals to psychiatry when appropriate.
• Conduct risk assessments utilizing structured interviews and risk assessment tools.
• Manage inmates at risk for suicide and/or engaging in self-harm according to existing protocols and current policy and procedures.
• Serve as the PCM mental health liaison for mental health services outside the facility.
• Facilitate admission and placement of inmates/patients to forensic hospitals when necessary.
• Monitor all mental health services provided.
• Ensure clinical services meet NCCHC standards and accepted psychiatric/psychological standards.
• Participate as a member of the Quality Assurance Committee.
• Develop and conduct in-service education programs according to the annual in-service calendar.
• Collect data and prepare monthly reports reflecting performance.
