About The Position

Professional Care Medical Practice P.C. is seeking a Licensed Mental Health Counselor (LMHC) or Licensed Social Worker (LMSW or LCSW) to serve as a Behavioral Health Professional within the medical department at the Allegany County Jail in Belmont, NY. This is a full-time, 40-hour per week position, operating Monday through Friday during normal business hours. The role involves providing comprehensive mental health care, including assessments, risk evaluations, treatment planning, and coordination of services within a correctional environment. The clinician will act as a liaison between the medical unit and facility staff, and will also be involved in quality assurance and in-service education programs. This position offers a challenging yet rewarding opportunity to serve a vulnerable and underserved patient population, with a focus on full-spectrum mental/behavioral health care, including substance abuse/MAT counseling and therapy.

Requirements

  • Must be licensed as a LMHC, LMSW, or LCSW in the state of New York.
  • Must hold a Master’s degree or greater in Social Work, Psychology, or related Human Service field with a concentration in clinical course work, training and/or experience.
  • Must hold a valid driver’s license & have reliable transportation.

Nice To Haves

  • Minimum of two years of clinical experience.
  • Familiarity with substance abuse/MAT (i.e., Medicated Assisted Treatment) counseling is preferred.

Responsibilities

  • Conduct clinical assessments based on referrals and self-referrals, including documentation of findings, mental status examination, case conceptualization, diagnosis, and treatment plan.
  • Make referrals to psychiatry when appropriate.
  • Conduct risk assessments utilizing structured interviews and risk assessment tools.
  • Manage inmates at risk for suicide and/or engaging in self-harm utilizing existing protocols.
  • Serve as the PCM mental health liaison for mental health services outside the facility.
  • Facilitate admission and placement of inmates/patients to forensic hospitals when necessary.
  • Monitor all mental health services provided.
  • Ensure clinical services meet NCCHC standards and accepted psychiatric/psychological standards.
  • Be a member of the Quality Assurance Committee.
  • Develop and conduct in-service education programs.
  • Collect data and prepare monthly reports reflecting performance.
